Trip/Fall LTI At Rugby Plant

Unfortunately, we suffered an employee LTI in Rugby Cement Plant when an employee parked his fork lift truck (FLT) tight up against a stack of palleted bagged cement (out of view of direct CCTV).  This location is not a designated parking area and the employee failed to reverse park, positioning his vehicle in a blind spot.  On exiting the

vehicle, the employee has reported that he tripped over a scaffold clip, injuring his right foot which resulted in a fracture of the 5th Metatarsal bone. There were no witnesses to the incident. We will share all learnings of the incident when investigation finishes.

With this incident we are closing June with two LTIs having now a total of four year to date, all of them in last two months. We need to reverse this trend quickly and come back to the performance we had in the first four months of the year, focusing all of us in Safety Essentials and showing a strong Safety Leadership making progress with our improvement plans that we have defined.
