We are sharing two Safety Alerts following incidents that occurred in EMEA, both specifically relate to machinery isolation. One of these incidents resulted in a life-changing injury; thankfully, the other resulted in no injury. However, the potential consequences in both cases were extremely serious, and the learning from each is equally important.

Please use both Alerts to reinforce the importance of effective isolation, identifying all sources of hazardous energy, and verifying that controls are in place before work begins.

It is vital that we learn not only from incidents where someone is injured, but also from those where a different set of circumstances could very easily have resulted in serious harm.
