In January we circulated an Alert detailing a high potential incident where 8 out of 12 mounting bolts securing a mixer truck drum to the gearbox were missing; it is a concern to have to circulate a second Alert on the same topic, relating to a very similar recent high potential incident, where only three remaining bolts secured a mixer truck drum in place.
This Alert highlights the importance of regular/robust inspection and defect reporting schemes in accordance with manufacturers’ recommendations, correct maintenance procedures and pre-use checks, requirements that are common to most of the equipment we operate.

Please take the opportunity to stress to your teams the benefit of reporting high potential incidents and the importance of carefully considering the learning points included in Safety Alerts, to identify whether there are any gaps in local safety practices and, if so, what action is required to reduce any shortfalls… let’s not wait for someone to suffer a serious injury before we learn from experience.
