The latest Safety Alert relates to a Contractor LTI that occurred in the UK when a train driver slipped on an icy cover while exiting a site after his shift, having deviated from the designated walkway during winter conditions.
Although the contractor was inducted, familiar with the site, and wearing suitable PPE, the incident highlights gaps in walkway definition, post-maintenance housekeeping, weather risk management, and timely incident reporting. You can find this Safety Alert on the UK News download page: here

This Alert can be used to remind drivers that loose items can become dangerous projectiles during sudden braking, sharp turns, or collisions, as well as using it to reinforce the need for cabs to be kept clean and tidy with items properly secured.
Please discuss this with your teams and the need to ensure access routes are clearly marked, maintained, left hazardfree after maintenance, and supported by clear safe systems of work, particularly during adverse weather.
