Latest Global Safety Alert

The latest Global Safety Alert relates to the death of an employee who was engulfed in material in a storage hopper, at a recently acquired quarry overseas. It is believed our colleague, who was 37 years old, climbed onto a conveyor which started suddenly, transporting him to the hopper.

This tragic incident underlines the importance of the work we have been doing this year to repeat the audits of machinery isolation arrangements and, more recently, our campaign to verify the understanding of isolation procedures of all personnel who are required to isolate machinery. Please discuss the Alert with your team(s), emphasising the importance of:

  • Only carrying out activities we are authorised and competent to undertake.
  • Robust isolation procedures (LOTOTO – Lock Out, Tag Out, Try Out).
  • Risk assessing the need for audible and visual prestart warning for conveyors and other moving machinery.
  • Taking the time to STOP & THINK / Take 5 before starting a new activity to ensure all necessary precautions are in place.
  • Always STEP IN / Take 5 Together if you see any activity where someone is at risk of injury.
  • Managing change… Whenever we bring new people or operations into the business do we ensure everyone is appropriately inducted / trained and assess equipment to verify it is safe.

Please ensure the ongoing isolation assessment campaign is completed thoroughly to confirm everyone who works with moving machinery demonstrates their knowledge and understanding of isolation arrangements through one-to-one discussions with their Line Manager in the workplace.
