Stop, Think And Check

It is very pleasing and encouraging to report on our 17th consecutive LTI.

I congratulate everyone for this performance that clearly shows that we, as a UK Team, can definitely deliver on our ZERO 4 Life commitment as long as we stay 100% focused and consistently demonstrates a safe behaviour and attitude every time, every where. Especially, we must stay alert and conscious of risks, always STEPPING IN when we perceive any unsafe behaviour or risk. Consistency and risk awareness form the corner stone of consistent safety performance.

However, we did record retrospectively an employee. One key element of reflection according to me is to always stop, think and check especially when any operations, even very simple, does not go as planned requiring some work-around (in this case, deciding “on the fly” to use a shovel to re-align the bolts). We should never allow ourselves to immediate work-around adoption. We should be more alert of inherent risks of “on the spot” reaction and should step back and take the required time to analyse the situation and make sure the operation is continued with proper operating model and tools. Let’s continue to insist on this matter that is one of the most frequent core root cause of incidents.
